I would rather go to a dude ranch or rafting down the Grand Canyon.
St Thomas and St John would be my pick. I have been to all 4 of them and the Sts are my favorite. St Thomas is a mini Oahu imo.

I lived in Puerto Rico back in the late 80s/early 90s. I'm sure it's changed a lot since then.
The people were very friendly and there were things to do: touring Old San Juan, casinos in the San Juan/Carolina area, Luquillo Beach, El Yunque Rainforest, snorkeling, scuba, etc.
It's a beautiful island but I think I'd rather be there in cooler weather.
I'm not sure how they've recovered from Hurricane Maria in 2017. I know they were really struggling for awhile but not sure now.
If you do go, plan a side trip of sorts to Vieques, which is an island off the east coast. It's more rustic and they have some cool things to do like a tour of the bioluminescent bay, snorkeling, etc.

Yes...COSTA RICA for sure. CR is actually pretty nice especially if you are into "eco" stuff. Look at going to the Volcano (arenal) area (La Fortuna is the town). Lots of hot springs, waterfalls, nature, etc.... It is a bit of a drive from San Jose, all dirt and crappy roads but once there it is convenient. Also very reasonable on the wallet. Me and my wife went for 4 days and it was around $700 for the flights and hotel (total, not each). We went just as "wet season" started (Aug) and it was very comfortable.

Done the grand canyon rafting. We went with one of the Indian reservation groups....just a day thing, not an overnight etc... My back could never handle that. It was "OK" but far less than expected. Keep in mind that I have done class 5 trips so it was very mild on our scale. Others might find it more enjoyable.
